OHSAA Physical Form

Don't be left out of the action! Have your OHSAA sports physical filed in the Athletic Office by July 27 in order to participate in camps or conditioning. Must have by August 3 in order to tryout for sports season.

COVID19 Return to Play Doctor Form

This form is required to be completed by a doctor once an athlete has recovered from a positive test for COVID-19 and their quarantine is over.
